	Medical Release Form This form MUST be completed and notarized before attending a youth ministry event. You are only required to fill one out per year, once we receive your form, we keep it on file. A copy of your insurance card is also needed on file.
	Scholarship Form Please turn in completed forms to Raul Hernandez, Youth and Family Minister.
	School Visit Form If you would like Raul to visit your child at school and have lunch with them, please complete this form and turn it in to Raul.
